ATHLETIC PARTICIPATION REQUIREMENTS

All students interested in participating in athletic events in Conway Public Schools are required to have a yearly physical.  In addition
to the physical, all athletes and parents are required to sign the consent form, found on page 3 of the Athletic Handbook.  The consent states that the student has permission to participate in athletics and you have read the Athletic Handbook and agree with the information in it.

INSURANCE INFORMATION

The Athletic Department has an insurance policy for all athletes
in the Conway Public Schools.  This insurance is a SECONDARY
insurance, meaning that it goes into effect after the primary insurance has considered the claim.

If an athlete is injured during an athletic event (practice or a game) the athlete needs to notify the coach immediately.  The coach will then complete an accident report, which is forwarded to the insurance company.  From this point, the insurance company will correspond directly with the parents.  It is the parent's responsibility to complete the required paperwork for processing.  A delay in supplying the required information may result in a denial of the claim.

The school district does not pay any of the expenses or process the paperwork.

If you do not have medical insurance, it is suggested that you look into ARKids or Medicare.
